Community Housing Solutions' Aging Gracefully in Place program
What do I get if I participate in this program?
• No-cost home visits from professionals who suggest help improvements to your home.
• No-cost minor home repairs.
• A $25 gift certificate each time you participate in a study evaluation visit.
What must I do?
• Over a four-month period, work with occupational therapist for six home visits and a study nurse for four to identify and improve problem areas with your physical abilities. You help set goals to work on. Each visit takes about an hour.
• Allow a contractor to do minor home safety repairs that you and the study occupational therapist identify.
• Allow three 45-minute study-evaluation visits to talk about your health and check your home. You'll be interviewed about your physical health, mental health and home safety. You do not need to answer any questions you do not want to. Answers are confidential.
• Allow a quick visual check for home safety hazards. Some photos may be taken, with your permission.
